Testosterone gel (Testogel)

About this product

Testosterone replacement therapy is licensed in the UK for the treatment of hypogonadism in men when testosterone deficiency has been confirmed biochemically (on at least two separate morning fasted samples between 7 and 11 AM, per BSSM 2017 UK guidelines) and clinically. The diagnosis must be supported by a recognised clinical picture of testosterone deficiency syndrome and excluded for reversible causes (medications, sleep apnoea, severe systemic illness). Pre-treatment workup typically includes total testosterone (two morning samples), SHBG and calculated free testosterone, LH and FSH to distinguish primary from secondary hypogonadism, prolactin, full blood count (baseline haematocrit), PSA in men over 40, liver function and lipid profile. Initiation, dose titration and ongoing monitoring follow a structured BSSM-aligned protocol including review at 3, 6 and 12 months and annually thereafter, with surveillance of haematocrit, PSA, lipids and clinical response. Section 4.3 contraindications per the Testogel and Tostran SmPCs are known or suspected prostate cancer and breast carcinoma. Testosterone should also be stopped immediately in patients with severe cardiac, hepatic or renal insufficiency due to the risk of oedema and congestive cardiac failure (Section 4.4 warning rather than Section 4.3 contraindication). Relative cautions include raised baseline haematocrit, untreated severe sleep apnoea, and active fertility intentions (TRT suppresses spermatogenesis). The prescribing clinician will discuss the licensed indication, the BSSM diagnostic framework, the realistic benefits and risks, the monitoring schedule, and the fact that this is a long-term medical commitment before any prescription is issued. How it works

Testosterone gel applied to clean, dry skin (shoulders or upper arms per the SmPC for the chosen brand) is absorbed transdermally and restores serum testosterone toward the normal adult male range. Restoration of physiological testosterone reverses the symptom cluster of testosterone deficiency syndrome (fatigue, mood, libido, body composition, bone mineral density). The gel formulation gives a more stable serum profile than older intramuscular preparations.
